The Hawaii Department of Health issued a violation notice and $315,000 fine to Nenita Mauricio for operating an unlicensed adult residential care home in Ewa Beach on Oahu.  

After receiving multiple complaints about an unlicensed adult residential care home, DOH inspectors went to 91-1046 Muiona Street to investigate. The inspectors confirmed six unrelated residents were receiving care at the home at different times, according to a DOH news release.

Mauricio, who is a certified nurse aide in Hawaii, admitted to inspectors that she was providing care for people in her home. 

DOH defines an adult residential care home as a facility that provides 24-hour living accommodations for a cost to adults, who require at least minimal help. Hawaii law requires these facilities to be licensed. 

As a result of the investigation, Mauricio is required to stop operating her unlicensed facility and pay a penalty of $315,000. The fine includes $1,000 for each day the unlicensed facility operated — a total of 315 days, starting on July 1, 2023. 

Mauricio has 20 days from the violation notice to submit a written request for a hearing. If Mauricio does not request a hearing, the violation notice will be final and enforceable.
