An inmate’s death at Monroe County Jail has triggered an investigation by the New York State Commission of Corrections.

The inmate, who has not been named, was found unresponsive September 5, one day after the incident, and later died at the hospital.

According to the Sheriff’s Office, an internal investigation found that a deputy conducted two incomplete rounds on the floor where the inmate was housed. That deputy faced “significant” discipline and has since been demoted.

The office said it has reinforced and communicated procedures to jail staff that were not followed that night.

Internal affair findings have since been shared with the inmate’s family.
