GOLDSBORO, N.C. - The workers killed and seriously hurt in a transformer accident have been identified. 

Officials say 51-year-old Dennis Martin of Goldsboro and 33-year-old Daniel Craig Anderson of Dudley were killed in an industrial accident Monday night. 

It happened at SPX Transformer Solutions off U.S. 117 in Goldsboro.

One person was working inside the transformer and became unresponsive. Two other workers went inside and became trapped. 

The third person, 40-year-old William Sariak of Dudley, is in critical condition at Wayne Memorial Hospital. 

Officials have not released details surrounding the incident; however, an extensive investigation is underway.

Investigators will look through evidence and interview employees as part of the process which could take up to six months to complete.

SPX Transformer Solutions, which employs about 1,000 people at the Goldsboro location, received a Silver Safety Award from the Department of Labor for the second consecutive year back in May, but the company has been under scrutiny before when an employee was electrocuted in 2012.

The Department of Labor’s Occupational Safety and Health Division says for this calendar year through October, they have investigated 35 work-related fatalities compared to 37 last year during the same time period. However, that statistic only reflects the work-related deaths in North Carolina that the agency is authorized to investigate.

SPX Transformer Solutions released a statement Tuesday afternoon:

“On the evening of November 30, 2015, an incident occurred at our SPX Transformer Solutions manufacturing facility in Goldsboro, North Carolina that resulted in the tragic deaths of two SPX employees and the serious injury of a third employee. We are shocked and deeply saddened by this incident, and offer our sincerest condolences to the families and loved ones of the employees involved. 

“The safety of our employees and their work environment is a top priority for SPX, and we are working closely with representatives of the Occupational Safety & Health Administration (OSHA) and other authorities to conduct a thorough investigation of this incident and its cause. We are also providing grief counseling and support services to all our employees at our Goldsboro facility.”
