CHAPEL HILL -- UNC Health Care is notifying its patients of a possible information breach.
They say prenatal patients between 2014 and 2017 may have filled out forms that were mistakenly transmitted to local county health departments.
The information retrieved includes addresses, social security numbers and health status.
Patients seen at the women's clinic at NC Women's Hospital and UNC Maternal-Fetal medicine at REX may have been affected.
A call center has been set up for patients who have questions or concerns. The toll-free number is 1-800-596-8362, and you can call to speak with someone Monday-Friday, 8 a.m. to 5 p.m.
